Social Prescribing

The Social Prescribing Service is designed to support patients with a wide range of social, emotional and practical needs. Recognising that health is determined by social, economic and environmental factors. Social prescribing seeks to address patient’s needs in a broad and holistic way. Some of the patients who access the service are worried about their financial situation or housing, feeling very lonely and isolated or stuck and unhappy.

Rachel Dawson, the Social Prescriber at Newbridge Surgery, works with patients over a short period of time to explore what most matters to them and through guided conversations, identifies solutions which will improve their general wellbeing. This often involves linking patients with services and activities in the community.

Patients 18 and over can access the service by booking a telephone triage  appointment.

Wellbeing Walks

Join our gentle walking group for short walks of up to 30 minutes in small groups.

Meet at Combe Down Surgery 1.45pm for a 2pm walk every other Wednesday

Where:

Combe Down Surgery, Combe Down House, The Avenue, Combe Down, Bath BA2 5EG 

Who:

Those interested in gentle exercise – people with health conditions such as obesity, or some mobility issues. Suits people experiencing anxiety about exercise or socialising. People who’d be interested in training as walk leaders are also welcome, and we can provide training and build confidence to do that.
